§170.315(b)(10) Electronic Health Information Export

BOLT extracts electronic health information (EHI) in accordance with the requirements of §170.315(b)(10) Electronic Health Information export as defined by the ONC 2015 Edition Cures Update Certification Criteria. EHI Exports will be available by December 31st, 2023.

Each export is packaged in a zip file, which includes documentation from the electronic health record. The BOLT EHI Export Tool is configured for the ability to restrict access to users who can perform the export. The EHI export can be done for either a single patient or a patient population, in an electronic and computable format. The electronic zip folder will contain files organized by sections, making it easy to locate the necessary health information and ensuring readability. The table below defines the content in each folder contained within the export zip file as applicable.

Content of the Export Zip File

Electronic ChartFolder contains all content within the EHI export in a machine readable XML format.
Provider Clinical NotesFolder contains clinical notes documented from a provider such as a Physician, Nurse Practitioner, Physician Assistant, Nurse Midwife, or Resident. Clinical notes include but are not limited to initial notes, visit notes, procedure notes, progress notes, and discharge notes for the inpatient and ambulatory setting. Documents are in a PDF format.
Nursing & Ancillary Clinical NotesFolder contains clinical notes documented from a healthcare team member in a nurse or ancillary role. This includes but is not limited to documentation from nursing, care management, physical therapy, speech therapy, occupational therapy, pharmacists, etc., in both the inpatient and ambulatory setting. Documents are in a PDF format.
Clinical Data SummaryThis folder contains a patient-level summary of clinical data in a PDF format, which includes: Alerts, Clinical Trials, Allergies, Home Medications, Immunizations, Preventions & Screenings, Ejection Fraction, Implants, Surgeries & Procedures, Social History, Pregnancies (if applicable)
Personal Data SummaryThis folder contains a patient-level summary of personal data in a PDF format, which includes: Patient Information (date of birth, gender, etc.), Demographics, Care Team, Sexual Orientation & Gender Identity, Biological Relationships, Communication Barriers, Advanced Directives, Patient Contacts, Patient Contact Notifications, Patient Address, Phone, Email, Employers, Preferred Pharmacies, Family History
Visits & AppointmentsThis folder contains a list of previous encounters & upcoming appointments in a PDF format.
Laboratory Results & ReportsThis folder contains laboratory results, radiology reports, microbiology results, and other reports in a PDF format.
ImmunizationsThis folder contains immunization data in PDF format.
MedicationsThis folder contains medication data in PDF format.
Communications & MessagesThis folder contains communications such as patient-provider messages and referrals in PDF format.

XML Documents

< Reports >All reports contained in the XML document will be inside the < Reports > tag.
< ExportRenderableReport >One per report is contained within the document. Every XML document will have at least one, and possibly several, < ExportRenderableReport > tags. Each < ExportRenderableReport > will contain a < ReportType >, < ReportDate >, and < ReportText > tag.
< ReportType >Title of the report.
< ReportDate >Date/Time the report was produces as part of the export process
< ReportText >The text of the report, in plain text format.
